The Detailed Procedures for
Receiving Hackathon Award Prizes

  1. Participant Qualification:
    The Hackathon is open to teams of eligible individuals. If a team is entering the hackathon, all must register on this form and note their team name. Each attendee must meet the eligibility requirements. The Hackathon is open to all individuals at least 13 years of age at the time of entry. The Hackathon is NOT open to individuals who are residents of a country, state, province, or territory where the laws of the United States or local law prohibits participating or receiving a prize in the Hackathon (including, but not limited to, Brazil, Quebec, Italy, Cuba, Sudan, Iran, North Korea, Syria, and any other country designated by the United States Treasury's Office of Foreign Assets Control). The Hackathon is NOT open to any individual whose participation in the Hackathon would create a real or apparent conflict of interest.
  2. Only registered participants are qualified to participate in the event and receive awards.
  3. If your team is winning an award, each qualified member in the team will share the award money equally together. Award Prizes are taxable incomes for the awardees. The final checks will be issued to the qualified individual team members.
  4. To receive the award prize, each winning team will email their team members’ name and email address to Dr. Tsou (mtsou@sdsu.edu) before 11/01/2024.
    * Additionally, teams that win 1st, 2nd, or 3rd place are required to create a 1-minute video showcasing their product and sharing their experience. Attach the video to the email referenced above.
  5. Dr. Tsou will email the "MISCELLANEOUS INCOME DISBURSEMENT REQUEST" form (Link) to all team members to fill out the form (adding your own addresses, email, RED-ID (for SDSU students), etc.) and then send the filled form back to Dr. Tsou.
  6. If the winning team member is an international student with F-1 Visa, the student need to provide the following document along with the Miscellaneous Income Disbursement Request form:

    •  Copy of visa and passport, and I-20.
    •  Foreign national Information Form (from SDSU Research Foundation website): (Dr. Tsou will provide the web link to fill out the form.)
    •  W-8BEN form (from SDSU Research Foundation website: (Link)
    •  8823 form if they have SSN or ITIN. (Link)

  7. If any winner team member does not provide the required documents and form by November 15, 2024 or without a reasonable explanation for delays. The award reimbursement procedure will be cancelled for this member due to the missing information.
  8. Winning team members will have opportunities to visit Sharp Prebys Innovation and Education Center (SPIEC) in November or December 2024. Sharp Prebys Innovation and Education Center (SPIEC) was built as an ambitious dream of innovation, education, and technology together in one space to promote research, training, and workforce development across Sharp HealthCare and the San Diego community. The building is over 70,000 square feet, this 4-story building includes an auditorium seating over 300 and is equipped with advanced multi-media capabilities to video conference from Sharp facilities throughout the county, ample conference and meeting space, a best-in-class simulation center, technology immersion lab, and technology demonstration space. Additionally, there is in-house catering, executive board rooms, as well as an outdoor terrace for social and networking events.
